Cancellation Policy
This agreement is made between the Driver named in the form above (hereinafter referred to as the “Client”) and Doctors Place INC (DPI) to outline the responsibilities and understanding for CDL drivers registering as an Owner-Operator (OWOP) in the FMCSA Clearinghouse to complete the Return-to-Duty (RTD) process as allowed under 49 CFR 382.705(b)(1)(ii). Clearinghouse Registration • I understand that as a CDL driver without a current or prospective employer, I must register in the FMCSA Clearinghouse under the "Employer" role for the limited purpose of completing the RTD process. • I will create a new Login.gov account and select "Register as an Employer and Driver", ensuring I select "Yes" for the question, "Are you an owner-operator?" Designation of a C/TPA • I understand that I must designate a consortium/third-party administrator (C/TPA) to facilitate my RTD process. • I authorize Doctors Place INC (DPI), my designated C/TPA, to direct me to complete the RTD test and to report the negative RTD test result to the Clearinghouse on my behalf. • Report ledge that once DPI reports my negative RTD test result to the Clearinghouse, my status will change from “prohibited” to “not prohibited,” allowing me to resume driving a commercial motor vehicle (CMV) and performing other DOT safety-sensitive functions. • I understand that my follow-up testing plan, as prescribed by the Substance Abuse Professional (SAP), must be administered by my future employer when I resume working in a DOT safety-sensitive position. Prohibition on U.S. DOT Number Application I confirm that I will not apply for a U.S. DOT Number solely for the purpose of completing the RTD process using this OWOP registration option. PROGRAM COST The cost for this program is $178, which includes account setup and deployment, coordination of the Return-to-Duty (RTD) process and Clearinghouse responsibilities, specimen collection fee, SAMHSA Lab fee, and MRO fees. As part of this service, Doctors Place INC will generate a DOT electronic Custody and Control Form (eCCF) and email it to the driver, along with clear instructions for completing the RTD test. The driver’s test will be scheduled at our Bothell, WA, location, provided the driver resides in Western Washington, ensuring the collection takes place at our designated collection site. Additionally, Doctors Place INC will report the negative RTD test result to the FMCSA Clearinghouse.
